It Begins:

I never expected for this project to be as popular as it has. What started as a time-saver for a chronically ill relative has suddenly gained new meaning in a pandemic world.

On asking the incredible community at reddit.com/askreddit it turned out there was a need for this beyond what I thought. Since relatives are no longer allowed to travel with patients the need for good information to hand to paramedic staff is more pressing. I've been fortunate to receive lots of feedback from professionals and individuals culminating in version 3.
